• IN-HOME SMART In the in-home component of the SMART program, trained volunteers visit on a weekly basis with clients to perform a series of 15 gentle exercises that are designed to help maintain independence. The exercises can be adapted for participants to any level, and helps build the muscles that are used every day. This component is offered to Brant, Haldimand and Norfolk residents. |

• GROUP EXERCISE SMART In the group component of the SMART program, two trained volunteers lead a weekly class of group participants. These classes are held in buildings where clients already live, or access regularly, thus removing the barrier of transportation. This program is offered to Brant residents in various locations across the county. |

The SMART program aims to help seniors stay independent longer and reduce the risk of falls. The regular social interaction with the volunteers and peers also helps clients who may feel lonely or isolated. Each week of the program costs $4, however if the client cannot afford to pay, financial assistance may be available. The SMART program is 100% funded by the HNHB Local Health Integration Network as part of the Aging At Home initiative.
